When Does Participant Heterogeneity Threaten Signal Detection in Psychiatric Trials?

Heterogeneity in psychiatric trials, or the natural variation among participants who ostensibly share a diagnosis, can complicate signal detection in CNS studies. When manageable sources of variability are not adequately addressed, they may increase measurement noise and placebo response, obscure true treatment effects, and make decisive evidence harder to generate. Targeted mitigation strategies can help protect signal detection while preserving appropriate real-world diversity.

The practical goal is not to eliminate diversity, but to identify and manage protocol-relevant sources of variability that could compromise interpretability.

Sponsors may reasonably be concerned that additional safeguards or more selective eligibility criteria could affect recruitment, timelines, statistical power, or external validity.
